For Repeat Prescriptions
To request your repeat prescription, you can:
- Fill out your prescription request form with the items clearly marked in reception or in the prescription box during our opening hours.
- Fill out a blank prescription request slip with your name, date of birth and the medication you require, and hand this into the surgery reception or drop in the prescription box.
- Submit a prescription request via our surgery email which is stelizabethsmc1.noreply@nhs.net when sending an email request please include your name, date of birth and the medication you require.
- Alternatively, you can post your request into the letterbox at the gates any time of the day or night. For Evington patients, prescription requests can be posted through the front door letterbox.
Please allow 48 hours for your prescription request to be processed.
Telephone requests will not be accepted.
Ordering Repeat Prescriptions through the NHS App
If you have an online account through airmid you can order your repeat prescriptions on the NHS App. Please speak to our reception staff to set up your online account.
If you regularly order your repeat prescriptions through the NHS App and it is no longer appearing – you may be due for a Medication Review with a member of our Clinical Pharmacy Team. In this event, please contact the surgery and we will arrange this for you.
